By filling out the form below, I hereby agree to abide by and follow Diocesan Policy for the Protection of Children: Code of Conduct (if 18 yrs of age or older), follow all Diocesan policies and practices, attend all formations preceding the TEC Retreat, participate in all activities of the formations and the weekend, and will notify the Lay Director and Chair of Team Selection Committee if I am unable to attend formations or cannot be on the weekend. I will not make copies of any music: in compact disk, tape or printed matter, play music in the conference room that has not been licensed, make copies of copy-written material without the written permission of the copyright holder, and I will not bring alcohol, tobacco, drugs, or weapons to any Southwest Indiana Teens Encounter Christ function.

If you have not had a Criminal Background Check and YPC Training, please contact your home parish to have these performed. You cannot work a team without these two requirements.
